For a checklist: Start with a question (which can be a statement or title for an upcoming series of check boxes) Then add the content you want to have check boxes to the left of to the options section. It was easy for me to modify the form so that each student sees a different question order or answer order which cuts down on cheating. The only down side for me was the use of the semi-colon to divide the multiple choice answers. Overall it serves its purpose!
